I would just write a paper on Microsoft Word (or any program you have) that says:
"I _______________ (insert name here) give permission to ______________ (Donald w/e your last name is
) to upload a video with me in it, or to sell a video with me in it, without suing me."
(Put the last part in more legal terms, but I have used this slip of paper and it works perfectly, with their signature on it, and the words clearly stated, you'll be fine
)

I just searched video waiver on google, and things came up. I found one that is used by a college for a commercial they were doing. And modified the existing waiver for the company name. If you need my version as a "rough draft" let me know, I would be happy to send it your way. There is more information that you need than what was already stated. Some stipulations like what the video can be used for.
Why not make a video??? If it fails at an early age then it is a lesson well learned. Plus it does wonders for a college resume to say that you published something, even if it fails. To quote from Finding Forrester (please modify for this purpose)*
