I was about to fill out the tax page which was sent to everyone to fill out but I have a few hesitations.

1. The first is the part where you add your social security number. I am not 100% sure that it is a good idea for me to enter it.

2. I don't have a bank account (I'm 14 years old) so I won't be able to use the check

Then you're stuck. Either ask your parents to open up your own bank account, or ask them to use their info.

everything is legit. You would need to fill out that form if you worked for theory11 anyway. Either you fill it out or you don't get paid.
